ALERT: BLOOD BANK TEST REQUESTS MUST COMPLY WITH THE FOLLOWING (in accordance with Federal requirements).
1. COLLECTOR INFORMATION is needed:
• The person that collected the blood (collector) from the patient must be documented
• The collector information can be provided on the test order/requisition (preferred)
or
be available in the EMR where the collection occurred
2. SPECIMEN LABEL for this testing MUST include:
• Patient full name (first and last name)
• Date of birth (DOB)
• Date and time of specimen collection
3. SPECIMEN ORDER/REQUISITION MUST match the specimen label information.
Specimen Handling:
• Pink or Lavender top: Collect. Mix gently after collection. Label. Submit in primary tube.
• Red top: Collect. DO NOT SPIN. Submit in primary tube.
